Sports Physicals

All students participating in an interscholastic sport; Modified, JV or Varsity, must have a current NYS School Health Examination (Sports Physical) on file, and the Dominick Murray Sudden Cardiac Arrest Prevention Screening on this form must be completed by the healthcare provider.

Students may have their health exam done with Bassett Healthcare School Based, if they are enrolled, or their own primary care provider and then fax (607-397-9454) or bring the exam form to the school's health office. Once completed the health exam covers the student for any sport for the next 12 months unless they are injured or seriously ill. In such a case, the student must provide a written release from their primary provider before returning to sports practice or games. ***

In addition to having a physical on file the following must be completed:

1. Under the Athletics Department tab click on Aktivate and create an account for the student athlete.

For the Health Office complete the following within 30 days of the first day of practice for each sport:

1. Custom Forms:
     A. Emergency Medical Release - you will need the student's date of last Tetanus vaccine. 
     B. NYSED Interval Health History for Athletics - you will need the student's date of last health exam. 
          **When completing the questions, they are to be answered for any illness/diagnosis/condition that 
          has occurred since the student's last health exam. If you answer "YES" to any question then an 
          explanation must be provided at the end of the questionnaire.
