NYS Physical Exam Requirements

Best practice for children grades Pre-K through 12th is to have annual physical exams.


NYS requires Physical Exams for all Pre-K, Kindergarten, 1st, 3rd, 5th, 7th, 9th and 11th grade students. 


In addition, all new entrants regardless of grade are required to have a physical exam for that school year on record within 30 days of entry. 


You are encouraged to have this done with either Bassett Healthcare School Based, if they are enrolled, or with your own Primary Care Provider. Once completed bring in a copy of the exam or have it faxed to 607-397-9454 for your child's school records. 


NYS Education Law effective 1/31/21, requires all Health Examinations performed for public school students to be documented on the Required NYS Health Examination Form.



If you have received a notice about this please be sure to complete and return it, to let me know where and when your child's physical exam will be done. If you have any questions regarding these requirements, contact me by email, [email protected] or phone, 607-397-8785 ext. 126.


Should the required health examination certificate not be provided to the school, you will be contacted regarding keeping your child home from school until the physical exam is received in the school's health office.


NYS also requires all new entrants and grades Pre-K, Kindergarten, 1st, 3rd, 5th, 7th and 11th grade to be screened for hearing and vision problems. Girls in grades 5th & 7th and boys in 9th grade to be screened for scoliosis. If these are not completed on their annual health exam form they will be performed by the School Nurse.  This is an ongoing effort throughout the school year. Whenever a student is tested and found to not meet minimal standards, a referral will be mailed to the parents. 



**Dental Exam Certificates are encouraged at the same time as Health Exams but are optional.
