NYS Law - Immunizations

Immunization Requirements


All students enrolled in a public school must be age-appropriately immunized. Immunization records should be submitted to the school’s health office prior to the student’s first day but no later than 14 days after beginning.



Students entering 6th grade must be immunized with the required Tdap (Tetanus, Diphtheria and Pertussis vaccine adolescent booster), which is given between the ages of 11 - 12 years. 

        ***Students that are not 11 years old and entering 6th grade are required to receive the  
              Tdap vaccine no more than 14 days from their 11th birthday.
  

Students entering 7th grade must have one dose of the Meningococcal vaccine given between ages 11 - 12 years (this is not the Meningococcal B vaccine). 


Students entering 12th grade must have 2 doses of the Meningococcal vaccine, the second dose given on or after their 16th birthday. 


**The above vaccines should be completed to start school in these grades. 


Medical Exemptions for Immunizations must be completed annually on the NYSDOH Medical Exemption Statement, by a Physician licensed to practice medicine in NYS.
