If your child is injured in an interscholastic sport, non-interscholastic or any school-sponsored activity, and you seek medical attention, be sure to notify the health office as soon as possible to make sure an incident report has been completed by the supervising school employee.
Pupils Benefits Plan Inc., our student accident insurance company will need a claim form which is provided by this office. This form will have the School Section completed, the parent/guardian will be responsible for completion of the Provider and Parent Sections and then mailing the form and all necessary paperwork to the address on the Pupil Benefits Plan form.
In most cases Pupils Benefits covers most, if not all, of what your insurance does not.
